Job Description ADULT SOCIAL CARE Experienced Mental Health Social Worker Camden Council

Duties:

Care coordination of complex cases under CPA

Developing coproduced care plans with service users

Risk assessment and management

liaison with inpatient units

accurate record keeping and report writing.

Experience:

Must have experience of working with adults with psychosis and complex needs.

Experience of Care Act assessments and reviews

Must have a good knowledge of relevant legislation policy & procedure including Safeguarding Mental Health Act Care Act and Mental Capacity Act
